Brigham and Women's Hospital

The Partners HealthCare Diabetes Management Program

The Partners HealthCare Diabetes Management Program, an affiliation of physicians, nurses, certified diabetes educators, and other clinical staff, provides resources to people living with diabetes in Eastern Massachusetts.  The Diabetes program is a Partners HealthCare system-wide collaboration to improve outcomes of people living with diabetes by promoting access to care, self-management through diet and exercise, community resources, and physician education.
